Program:
The conference will open on Sunday, November 8 with a keynote address by Donald M. Berwick, MD, MPP, President and CEO of the Institute for Healthcare Improvement (IHI). Dr. Berwick is also Adjunct Associate Professor of Health Policy and Management at the Harvard School of Public Health, and has published more than 80 scientific articles on subjects relating to health care policy, decision analysis, technology assessment, and health care quality management.

Dr. Berwick will report on lessons learned from these promising and pioneering efforts, including two recently completed "Breakthrough Series Collaboratives" of the Institute for Healthcare Improvement, chaired by Dr. Lucian L. Leape, and focusing on reducing medical errors and adverse drug events.

The rest of the conference program will feature plenary sessions on "Creating a Culture of Safety," "The Scientific Investigation of Avoidable Patient Injury: Two Approaches," "Improving the Reporting and Analysis of Incidents," and "Responses from Multiple Perspectives." The program also will include three case studies: "The Need to Blame and the Assessment of Negligence and Incompetence," "Irreducible Error Rate: The Pap Smear Scenario," and "Sentinel Event Case Study and Critique of an Imperfect Root Cause Analysis." Finally, the program will include three breakout sessions on a range of patient safety topics, featuring presenters selected through the abstract review process.
