The value of currently available antiretroviral drugs has been severely limited by the extraordinary mutability of HIV, which has also proven to be very adept at evading the induction of protective immunity via immunization. The development of effective therapies against HIV, therefore, is going to be dependent on novel strategies, including approaches based on models of pathogenesis, infection and replication that run counter to common beliefs. The selection of new targets that inherently limit the potential for drug resistance to develop, or new gene therapy or immunological approaches, may prove to be critical.
